Notice of Privacy Practices (HIPAA)
This notice describes how health information about you may be used and disclosed, and how you can get access to this information. Please review it carefully.
Jon B. Pease, MA, LMFT, at Accel Counseling Corp is required by law to: (1) maintain the privacy of your protected health information (PHI); (2) provide you with this Notice of Privacy Practices; and (3) follow the terms of this Notice.
Uses and Disclosures for Treatment: Your PHI may be used and disclosed for treatment purposes, including coordinating care with other healthcare providers involved in your treatment, only with your written authorization or as otherwise permitted by law.
Uses and Disclosures Requiring Your Authorization: Most other uses or disclosures of your PHI require your written authorization. You may revoke your authorization at any time in writing.
Your Rights: You have the right to request restrictions on uses and disclosures, request confidential communications, access your PHI, and file a complaint. To exercise any of these rights, contact: jon@accelcounseling.com
Complaints: If you believe your privacy rights have been violated, you may file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights, at hhs.gov/hipaa. You will not be penalized for filing a complaint.
Good Faith Estimate — No Surprises Act
Under the law, you have the right to receive a Good Faith Estimate explaining how much your mental health care will cost before you receive services.
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees. Make sure your healthcare provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your healthcare provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. For questions or more information about your right to a Good Faith Estimate, visit cms.gov/nosurprises.

Telehealth Disclosure
Accel Counseling Corp offers telehealth (video and phone) services to California residents. Telehealth involves the delivery of behavioral health services using electronic communications and may include audio-only or video sessions.
By engaging in telehealth services, you understand that: (1) you are responsible for maintaining a private, secure connection; (2) there are potential risks to privacy despite reasonable safeguards; (3) telehealth may not be appropriate for all clinical situations; and (4) you must be physically located in California at the time of service.
